An exciting opportunity exists for a dedicated and skilled Registered Nurse with strong home health experience to provide exceptional care to patients in their homes. This full-time, 13-week contract position offers a rewarding field-based role where you'll make a meaningful difference by delivering individualized nursing services and coordinating care as part of an interdisciplinary team.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Role Highlights:

  • Serve a typical caseload of 25–32 points per week, averaging 3–5 patient visits daily
  • Daily travel required within the assigned territory; mileage reimbursement provided
  • Utilize Home Care Home Base (HCHB) for all electronic documentation
  • Conduct OASIS assessments, care plan development, and ongoing patient evaluation

    • Key Responsibilities:

  • Perform comprehensive patient assessments and develop individualized plans of care
  • Deliver skilled nursing care including wound management, medication administration, IV therapy, and chronic disease management
  • Monitor patient progress, identify changes in condition, and communicate effectively with physicians and care teams
  • Educate patients and families regarding disease processes, medications, and overall care
  • Maintain accurate, timely documentation and participate in meetings, case conferences, and continuing education
  • Collaborate with therapists, medical social workers, and home health aides for comprehensive patient care
  • Adhere to regulatory and agency standards

    • Qualifications:

  • Current RN license in good standing (state of practice)
  • Minimum 1 year of recent home health nursing experience (field strongly preferred)
  • Proficient with HCHB and experience completing OASIS assessments
  • Excellent clinical assessment, organizational, and communication skills
  • Reliable transportation, valid driver’s license, and auto insurance
  • Ability to work independently and manage a flexible field-based schedule
  • BSN preferred; knowledge of Medicare and home health compliance standards is a plus
