Provide compassionate, patient-centered care to individuals at the end of life as a Hospice Registered Nurse (RN) within home and residential environments. In this contract position, you will leverage your clinical expertise and empathy to manage all aspects of hospice services, providing comfort and support to patients and their loved ones. You will collaborate with an interdisciplinary team to craft personalized care plans while upholding the highest standards of hospice nursing.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Occasional weekend or on-call coverage as needed
  • Key Responsibilities:

  • Conduct in-depth nursing assessments and develop individualized hospice plans of care
  • Deliver direct nursing interventions, focusing on symptom management and comfort
  • Administer medications and treatments per physician directives and hospice policy
  • Monitor and document changes in patient condition, communicating updates to the care team
  • Educate patients and families on disease processes, comfort options, and emotional support
  • Work collaboratively with social workers, chaplains, aides, and volunteers to address holistic care needs
  • Maintain accurate documentation in Home Care Home Base (HCHB)
  • Participate in case conferences, team meetings, and quality assurance initiatives
  • Uphold hospice care policies, standards, and state/federal regulations
  • Required Qualifications:

  • Valid RN license in good standing
  • Minimum of 1 year recent hospice nursing experience
  • Proficiency with Home Care Home Base (HCHB)
  • Sound clinical assessment and communication skills
  • Strong background in end-of-life care, symptom management, and family support
  • Reliable personal transportation, valid driver’s license, and current auto insurance
  • Ability to self-manage a field-based caseload
  • Compassionate, resilient, and adaptable demeanor
  • Preferred:

  • BSN degree
  • Experience in palliative care
  • Knowledge of Medicare hospice guidelines
