Deliver exceptional patient care as a Home Health Registered Nurse, providing personalized skilled nursing services in a field-based environment. This opportunity requires clinical expertise, autonomy, and a commitment to supporting patients’ health in their homes.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Key Responsibilities:

  • Conduct OASIS assessments, develop individualized care plans, and document accurately in Home Care Home Base (HCHB).
  • Deliver nursing interventions including wound care, IV therapy, medication administration, and chronic disease management.
  • Monitor and evaluate patients’ progress, promptly communicate any condition changes to the care team and physicians.
  • Educate patients and families regarding disease management, medication, and preventive care strategies.
  • Coordinate with multidisciplinary teams—including therapists, social workers, and home health aides—to assure continuity of care.
  • Participate in case conferences, meetings, and ongoing education.
  • Comply with agency procedures and all regulatory standards for excellent home health care delivery.
  • Daily travel within the assigned territory (mileage reimbursement is provided).

    • Qualifications:

  • Active Registered Nurse license in good standing for the state of practice.
  • Minimum 1 year of recent, relevant home health nursing experience (field experience preferred).
  • Proficiency with Home Care Home Base (HCHB) and OASIS is required.
  • Strong clinical judgment, critical thinking, organizational, and time management abilities.
  • Valid driver’s license, reliable vehicle, and current auto insurance.
  • Ability to independently manage a field-based caseload of 25–32 points per week.
  • Excellent communication and interpersonal skills.
  • BSN and prior experience managing larger caseloads preferred.
  • Understanding of Medicare and compliance standards for home health.
