As an RN specializing in Case Management in this dynamic location, you’ll play a pivotal role in optimizing patient outcomes and supporting transitions of care. Collaborating with interdisciplinary teams, you’ll be exposed to a variety of workplace cultures and leadership styles, propelling your professional growth while providing vital support to patients and families.

Qualifications & Desired Experience:

  • Current and active RN license (California)
  • Minimum of 1-2 years’ experience in Case Management (acute care setting preferred)
  • Proven ability to assess, plan, coordinate, and evaluate care for individuals across diverse populations
  • Strong knowledge of utilization review, discharge planning, and healthcare regulations
  • Excellent communication and organizational skills
  • Key Responsibilities:

  • Assess and manage patients’ transition of care needs from admission through discharge
  • Collaborate with physicians, social workers, and other health professionals to develop individualized care plans
  • Conduct utilization reviews to ensure appropriate resource use and quality patient outcomes
  • Advocate for patient needs, facilitating referrals and community resources as necessary
  • Maintain comprehensive documentation and participate in quality improvement initiatives
