Schedule and Hours:

  • 5x8-hour day shifts
  • Every other weekend required
  • Qualifications:

  • Active RN license required
  • Minimum of 2 years' recent acute inpatient case management experience is essential
  • Strong clinical assessment, care planning, and patient advocacy skills
  • Expertise in motivational interviewing, self-management, and critical thinking
  • Experience coordinating complex care across the continuum, especially following critical events or new diagnoses
  • Excellent communication abilities and a collaborative approach to care
  • Key Responsibilities:

  • Conduct thorough assessments of patient and family needs—including biomedical, psychological, social, and functional aspects—to inform recovery plans
  • Develop and implement individualized patient-centered care plans for optimal outcomes
  • Foster patient and family engagement, advocacy, and communication regarding health education and access to resources
  • Utilize professional judgment to counsel on treatment decisions, care transitions, and sensitive end-of-life issues such as advanced care planning
  • Provide seamless coordination during transitions between care settings
  • Collaborate with care teams to address complex medical and social needs, including facilitating interdisciplinary care conferences
  • Monitor, evaluate, and revise care plans as needed to ensure quality outcomes
  • Participate in initiatives targeting risk reduction and quality improvement
  • Address concerns related to abuse, neglect, or violence, intervening as appropriate
