Schedule:

  • Contract position with potential extensions

    • Qualifications:

  • Current and active LPN license
  • At least one year of recent experience providing home health care
  • Hands-on proficiency with HomeCare HomeBase (HCHB) EMR
  • Solid clinical assessment and documentation capabilities
  • Dependable transportation for reliable patient visits
  • Excellent self-management and organizational skills to oversee a caseload

    • Responsibilities:

  • Deliver skilled nursing services to patients based on individualized care plans
  • Continuously assess patient health, monitoring and documenting changes while updating care plans as needed
  • Administer prescribed medications and treatments in accordance with state regulations
  • Educate patients and their families regarding ongoing care management and illness prevention
  • Precisely maintain documentation and records using the HomeCare HomeBase (HCHB) EMR system
  • Work closely with registered nurses, therapists, and other healthcare team members to ensure comprehensive, coordinated care
