Schedule/Hours:

  • Contract assignment with potential for extension

    • Qualifications:

  • Active, unrestricted LPN license
  • At least one year of recent experience in a home health setting
  • Proficient in HomeCare HomeBase (HCHB) EMR
  • Strong clinical assessment and documentation abilities
  • Reliable transportation for patient home visits
  • Ability to manage time and case loads effectively, working independently

    • Primary Responsibilities:

  • Deliver comprehensive nursing care in patient homes following individualized care plans
  • Track, document, and update patient progress accurately using HCHB
  • Administer prescribed medications and treatments in accordance with state guidelines
  • Provide patient and family education to promote health management and disease prevention
  • Collaborate with RNs, therapists, and other healthcare professionals to ensure continuity and quality of care
