Schedule/Hours:

    • Qualifications:

  • Current, active LPN license
  • Minimum of one year recent experience in home health
  • Strong proficiency with HomeCare HomeBase (HCHB) EMR
  • Solid clinical assessment and documentation skills
  • Reliable transportation for in-home patient visits
  • Self-motivated with the ability to manage an independent caseload

    • Responsibilities:

  • Deliver skilled, individualized nursing care according to established care plans
  • Administer medications and perform treatments within LPN scope of practice
  • Monitor and document patient progress, updating care plans as needed
  • Educate patients and their families on care management and preventive strategies
  • Maintain thorough and accurate records using HCHB EMR
