If you have a passion for delivering exceptional care in a home-based setting, this Home Health Registered Nurse opportunity is tailored for you. This full-time, 13-week contract position is ideal for a self-motivated and compassionate RN who excels at independent work and prioritizes high-quality, patient-centered care.

Schedule/Hours:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Responsibilities:

  • Conduct thorough OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management in patient homes.
  • Monitor patient status, promptly communicate changes, and coordinate care with physicians and interdisciplinary team members.
  • Educate patients and families on disease processes, treatment, and preventive health.
  • Complete accurate, timely clinical documentation in Home Care Home Base (HCHB).
  • Foster collaboration with therapists, social workers, and aides to support optimal patient outcomes.
  • Participate in case conferences, required meetings, and continuing education.
  • Ensure compliance with regulatory standards and agency policies.

    • Qualifications:

  • Active RN license in good standing.
  • Minimum 1 year of recent field-based Home Health RN experience.
  • Proficiency with Home Care Home Base (HCHB) and OASIS documentation is required.
  • Reliable transportation, valid driver’s license, and auto insurance.
  • Excellent assessment, organization, and communication skills.
  • Ability to work independently and manage a flexible field-based caseload (25–32 points/week).
  • BSN and prior experience managing a full home health caseload are preferred.
  • Knowledge of Medicare and home health compliance standards is a plus.
