Deliver meaningful care where it matters most as a Home Health Registered Nurse providing service to patients where they feel most comfortable—their own homes. As an experienced, patient-centered clinician, you'll ensure the highest standards of nursing care while building trust and supporting independence for those you serve.

Schedule:

  • Monday through Friday, 8:00 AM – 5:00 PM
  • Full-time, 13-week contract
  • Caseload of 25–32 points weekly (3–5 patient visits per day)
  • Daily travel within your assigned territory (mileage reimbursement included)

    • Key Responsibilities:

  • Complete comprehensive patient assessments (OASIS) and develop personalized care plans.
  • Deliver skilled nursing interventions: wound care, IV therapy, medication administration, and chronic disease management.
  • Reassess patients’ progress, promptly communicating changes to the interdisciplinary care team and physicians.
  • Educate patients and family members on disease management, medication, and preventive practices.
  • Maintain accurate, timely electronic records in Home Care Home Base (HCHB).
  • Collaborate with allied health professionals to ensure coordinated care delivery.
  • Engage in case conferences, staff meetings, and continuing education opportunities.
  • Uphold regulatory and organizational standards for documentation and patient care.

    • Qualifications:

  • Active RN license (in good standing)
  • Minimum 1 year of recent home health RN experience, preferably with field-based background
  • Proficiency in HCHB and OASIS required
  • Strong assessment, critical thinking, and independent decision-making abilities
  • Valid driver’s license, reliable transportation, and proof of auto insurance
  • Excellent time management, organization, and communication skills
  • BSN and prior experience managing 25–32 points weekly preferred
  • Familiarity with Medicare and home health compliance a plus
