An exciting opportunity is available for an experienced and compassionate Registered Nurse specializing in home health to provide high-quality care to patients in a supportive field-based environment. This full-time role is structured as a 13-week contract and is well-suited for independent, organized professionals who thrive on promoting patient well-being directly in their homes.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Field-based; daily travel to patient homes within an assigned territory
  • Caseload: 25–32 points per week (typically 3–5 patient visits daily)

    • Qualifications:

  • Active RN license (in good standing) in the state of practice
  • Minimum of 1 year of recent home health nursing experience (field experience strongly preferred)
  • Proficiency with Home Care Home Base (HCHB) electronic documentation
  • Direct OASIS assessment experience
  • Reliable transportation, valid driver’s license, and active auto insurance
  • Solid clinical assessment, time management, and organizational skills
  • Excellent communication and ability to work autonomously
  • BSN preferred
  • Experience managing 25–32 home health points weekly and knowledge of Medicare/home health compliance are beneficial

    • Role Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized care plans
  • Deliver skilled nursing care: wound care, medication administration, IV therapy, chronic disease management
  • Monitor patient progress, identify and communicate changes promptly, and ensure timely documentation
  • Educate patients and families on diagnoses, treatment plans, medications, and preventive care
  • Work closely with an interdisciplinary team—including therapists and home health aides—for coordinated care
  • Attend case conferences, engage in staff meetings, and participate in ongoing education
  • Uphold regulatory and agency care delivery standards
