In this per diem role, you will directly support care continuity by bringing clinical expertise and compassion to patients in their residences throughout Dent County, including Salem, Bunker, and surrounding communities.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling—choose shifts that work best for you
  • Minimum weekly or bi-weekly hourly commitment
  • Key Responsibilities:

  • Conduct comprehensive annual and post-discharge risk assessments in patients’ homes
  • Complete each evaluation within approximately 45 minutes
  • Review and assess current and prior medical history, conditions, and medication profiles
  • Educate patients about chronic conditions and the proper use of medications
  • Identify care gaps and recommend appropriate interventions; no prescription or treatment plan required
  • Deliver detailed and supportive documentation of visits
  • Qualifications:

  • Active Nurse Practitioner (NP) license in Missouri
  • MSN from an accredited nursing program
  • Current BLS CPR certification
  • Board Certification through AANP or ANCC
  • Minimum of 6 months of NP clinical experience (preferred)
  • This role offers $80–$90 per completed evaluation, in addition to a mileage stipend and administrative time compensation, reflecting the commitment to recognizing your dedication and expertise.
