This per diem opportunity empowers Nurse Practitioners to deliver personalized care by conducting risk assessments for Medicare Advantage plan members in Shoshone County, ID. Visits may involve annual health assessments or follow-up post-discharge, with each evaluation averaging around 45 minutes in the patient’s home.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule
  • Minimum weekly or bi-weekly hourly commitment required
  • Primary Duties:

  • Perform comprehensive home health risk assessments for a Medicare Advantage health plan
  • Review and evaluate patients’ medical history, existing conditions, and medication use
  • Educate patients on managing medical conditions and proper medication use
  • Detect care gaps and provide recommendations to help resolve them
  • No responsibility for developing treatment plans or prescribing medications
  • Qualifications:

  • Active Nurse Practitioner License in Idaho
  • Current CPR/BLS certification
  • Board certification by AANP or ANCC
  • Minimum of 6 months of recent Nurse Practitioner experience is preferred
  • MSN from an accredited nursing program
