Deliver high-impact clinical care with the flexibility and autonomy of a per diem schedule as a Nurse Practitioner focusing on home health assessments across Madison County, Illinois. In this rewarding role, you will provide home-based risk evaluations for Medicare Advantage members, contributing to preventive care and improved health outcomes without the need for ongoing treatment planning or medication management.

Schedule:

  • Flexible shifts available any day of the week (Monday through Sunday)
  • Customize your own schedule to balance personal and professional commitments
  • Minimum weekly or bi-weekly hourly commitment required
  • Key Responsibilities:

  • Conduct comprehensive annual and post-discharge risk assessments in patients’ homes
  • Review and evaluate prior and current medical history, conditions, and medication regimens
  • Educate members on their medical conditions and prescribed medications
  • Identify gaps in care and offer targeted clinical recommendations for improvement
  • Visits last approximately 45 minutes each, providing time for meaningful patient interaction
  • No responsibility for developing treatment plans or prescribing medications
  • Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Valid Board Certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • Current BLS-certified CPR credentials
  • At least 6 months of experience as a Nurse Practitioner preferred
  • Enjoy a competitive per-evaluation compensation of $80-$90, with additional mileage and administrative stipends to support your work.
