Experience meaningful patient engagement as a Nurse Practitioner specializing in home health risk assessments throughout Stark County, Ohio. In this per diem role, you’ll perform comprehensive health assessments for Medicare Advantage plan members in their homes, covering destinations such as Canton, Massillon, North Canton, and Alliance. Visits last approximately 45 minutes, with flexible scheduling based on your availability.

Schedule:

  • Choose your own shifts between Monday and Sunday
  • Minimum weekly or bi-weekly hourly commitment
  • Adaptable schedule to fit your personal and professional life
  • Key Responsibilities:

  • Complete annual health risk assessments for Medicare Advantage plan participants in their homes
  • Examine medical history, review current/past medical conditions, and assess medication use
  • Educate members on their health conditions and prescribed medications
  • Detect gaps in care and make recommendations to address them
  • Note there is no requirement for treatment plan development or medication prescribing
  • Qualifications:

  • Valid Nurse Practitioner license in Ohio
  • Current BLS/CPR certification
  • Board Certification as an NP through AANP or ANCC
  • At least 6 months of recent Nurse Practitioner experience (preferred)
  • Master of Science in Nursing (MSN) from an accredited program
