Deliver compassionate and impactful care as a Home Health Licensed Practical Nurse (LPN) supporting patients in their homes. You'll play a vital role in promoting patient independence, educating families, and contributing to recovery—all while collaborating closely with a dedicated healthcare team.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM, with occasional flexibility to meet patient needs

    • Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • At least 1 year of experience in home health, hospice, or a comparable community-based care setting
  • Proficiency with Home Care Home Base (HCHB) required
  • Valid driver’s license, reliable transportation, and current vehicle insurance
  • Current BLS (Basic Life Support) certification
  • Strong clinical judgment, time management, and communication skills
  • Ability to work independently, managing a caseload in a field-based environment

    • Preferred Experience:

  • Previous work with Medicare-certified home health agencies
  • In-depth knowledge of home health compliance and documentation requirements
  • Excellent interpersonal skills with a patient-first approach

    • Key Responsibilities:

  • Provide skilled nursing visits, assessing and implementing individualized care plans for patients at home
  • Administer medications, manage wound care, and perform clinical interventions such as IV therapy as needed
  • Monitor and report changes in patient conditions to the RN Case Manager or physician
  • Educate patients and their caregivers on condition management, medications, and self-care techniques
  • Accurately document assessments and care using Home Care Home Base (HCHB)
  • Collaborate with the interdisciplinary team to coordinate and ensure quality patient care
  • Uphold infection control and safety standards at all times
  • Participate in case conferences, quality improvement activities, and ongoing training
