Deliver compassionate, high-quality hospice care as a Registered Nurse supporting patients in their homes or residential facilities. This contract role is ideal for an RN with expertise in both hospice and home health, who is committed to providing comfort, dignity, and peace to individuals facing terminal illness.

Schedule:

  • Full-time, 13-week contract
  • Monday – Friday, 8:00 AM – 5:00 PM
  • Occasional weekend or on-call rotation may be required
  • Key Responsibilities:

  • Perform thorough nursing assessments and develop individualized hospice care plans
  • Deliver direct skilled care to manage symptoms and ensure patient comfort
  • Administer medications, treatments, and therapeutic interventions per physician orders and hospice protocols
  • Monitor and report patient condition changes promptly to the care team and medical director
  • Educate patients, families, and caregivers on disease progression, effective comfort measures, and medication administration
  • Collaborate closely with interdisciplinary team members, including social workers, chaplains, aides, and volunteers, to address diverse patient needs
  • Maintain timely, accurate documentation using Home Care Home Base (HCHB)
  • Participate in team meetings, case conferences, and quality improvement initiatives
  • Adhere to all hospice policies and regulatory requirements
  • Required Qualifications:

  • Active RN License (in good standing) in South Carolina
  • At least 1 year of recent hospice experience
  • Proficiency with Home Care Home Base (HCHB) required
  • Excellent assessment, communication, and family support skills
  • Solid understanding of pain management and end-of-life care principles
  • Reliable transportation, valid driver’s license, and auto insurance
  • Ability to work independently in a field-based setting
  • Compassionate, emotionally resilient, and patient-centered approach
  • Preferred Qualifications:

  • BSN degree
  • Experience in palliative or end-of-life care
  • Familiarity with Medicare hospice regulations
