An immediate opportunity is available for an experienced and compassionate Registered Nurse dedicated to quality home health care. This contract position focuses on delivering skilled nursing services to patients in their residences, ensuring both clinical excellence and meaningful patient engagement.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Key Details:

  • 13-week contract
  • Field-based (patient homes)
  • Caseload: 25–32 points per week (approximately 3–5 patient visits per day)
  • Uses Home Care Home Base (HCHB) for documentation and OASIS assessments
  • Daily travel within assigned territory (mileage reimbursement provided)

    • Responsibilities:

  • Perform comprehensive OASIS assessments and develop individualized patient care plans
  • Deliver skilled nursing care such as wound care, IV therapy, medication administration, and chronic disease management
  • Evaluate patient progress and communicate condition changes promptly to the interdisciplinary care team and physicians
  • Educate patients and families regarding disease processes, treatment options, and preventive care
  • Accurately document all care activities in Home Care Home Base (HCHB)
  • Collaborate with PT/OT, medical social workers, and home health aides for coordinated care
  • Participate in case conferences, staff meetings, and ongoing education
  • Maintain compliance with agency and regulatory documentation and care standards

    • Requirements:

  • Active RN license (in good standing) for the state of practice
  • At least 1 year of recent, relevant home health experience (field experience preferred)
  • Proficient in Home Care Home Base (HCHB) and experienced with OASIS documentation
  • Excellent clinical assessment, organizational, and communication skills
  • Reliable transportation, driver’s license, and auto insurance
  • Ability to work independently

    • Preferred Qualifications:

  • BSN
  • Experience managing caseloads of 25–32 points per week
  • Understanding of Medicare and home health compliance
