This contract opportunity focuses on supporting individuals in their recovery and ongoing health management while emphasizing patient education and multidisciplinary collaboration. Schedule:

  • Full-time, 13-week contract
  • Monday – Friday, 8:00 AM – 5:00 PM
  • Caseload: 25–32 points per week (approximately 3–5 home visits per day)

    • Responsibilities:

  • Conduct comprehensive OASIS assessments and establish individualized plans of care.
  • Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, report significant changes to the care team, and update plans as needed.
  • Educate patients and their families about health conditions, medications, treatments, and preventive strategies.
  • Ensure prompt and accurate documentation through Home Care Home Base (HCHB).
  • Collaborate with physical therapy, occupational therapy, social work, and home health aide professionals to ensure coordinated, holistic care.
  • Participate in case conferences and ongoing educational opportunities.
  • Maintain compliance with all agency and regulatory documentation standards.

    • Qualifications:

  • Active and unrestricted RN license in the state of practice.
  • At least 1 year of recent, relevant Home Health nursing experience.
  • Proficiency with Home Care Home Base (HCHB) and OASIS documentation.
  • Clinical assessment, critical thinking, and organizational excellence.
  • Reliable transportation, current driver’s license, and valid auto insurance.
  • Ability to work autonomously and manage field-based scheduling.
  • BSN and knowledge of Medicare/home health compliance preferred.
