A rewarding opportunity is available for an experienced and compassionate Registered Nurse with a background in home health. This full-time, 13-week assignment involves providing high-quality nursing care to patients in their homes, ensuring each patient receives individualized attention and support. The role requires a self-motivated and organized RN who excels at clinical care, documentation, and patient communication.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Caseload: 25–32 points per week (typically 3–5 patient visits daily)
  • Field-based position with daily travel within an assigned territory (mileage reimbursement provided)
  • Key Responsibilities:

  • Perform OASIS assessments and develop individualized care plans tailored to each patient’s needs.
  • Deliver skilled nursing care including wound management, medication administration, IV therapy, and chronic disease management.
  • Evaluate and monitor patient progress, promptly communicate changes to care teams and physicians.
  • Educate patients and their families about diagnoses, treatment plans, medications, and preventive health measures.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB).
  • Coordinate with therapists, social workers, and home health aides to support unified patient care.
  • Attend case conferences, staff meetings, and engage in ongoing education as required.
  • Ensure compliance with all regulatory and agency clinical standards.
  • Qualifications:

  • Active RN license in state of practice, in good standing
  • Minimum one year of recent home health nursing experience (field experience preferred)
  • Proficiency in Home Care Home Base (HCHB) and with OASIS assessments required
  • Reliable transportation, valid driver’s license, and auto insurance
  • Excellent communication, critical thinking, and organization skills
  • BSN preferred and experience handling 25–32 points/caseload beneficial
  • Knowledge of Medicare and home health regulations preferred
