Deliver high-quality care and make a profound impact as a Home Health Registered Nurse (RN) serving patients in their homes. This contract opportunity offers the chance to use your clinical expertise and compassionate approach, working independently to support recovery and enhance quality of life.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Key Responsibilities:

  • Conduct thorough OASIS assessments, develop and implement individualized care plans.
  • Deliver skilled nursing including wound care, medication administration, IV therapy, and managing chronic illnesses.
  • Evaluate patient progress, address changes in condition, and coordinate communication with care teams and physicians.
  • Provide patient and family education regarding disease management, medications, and preventive care.
  • Accurately document all care using Home Care Home Base (HCHB).
  • Work closely with physical therapists, occupational therapists, medical social workers, and home health aides to ensure seamless, coordinated care.
  • Attend case conferences, staff meetings, and engage in continuing education as required.
  • Ensure care delivery and documentation comply with agency and regulatory standards.

    • Qualifications:

  • Active RN license in the state of practice (unrestricted)
  • At least 1 year of recent Home Health RN experience (field experience strongly preferred)
  • Proficient with Home Care Home Base (HCHB) and OASIS assessments
  • Proven clinical assessment and critical thinking abilities
  • Reliable transportation, valid driver’s license, and auto insurance
  • Excellent communication, organization, and time management skills
  • Comfort working independently and managing a field-based schedule
  • BSN preferred
  • Experience managing a caseload of 25–32 points weekly and knowledge of Medicare/home health compliance is a plus

    $1,970 - $2,084/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
