Contribute your expertise as a Home Health Registered Nurse by delivering skilled, compassionate care directly to patients in the comfort of their homes. In this field-based contract position, you’ll play a vital role in supporting positive patient outcomes—while exercising autonomy, developing individualized care plans, and collaborating with an interdisciplinary team.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Contract Details:

  • Full-time, 13-week contract
  • Target caseload: 25–32 points per week (typically 3–5 patient visits per day)
  • Daily travel within your assigned territory (mileage reimbursement provided)

    • Key Responsibilities:

  • Complete comprehensive OASIS assessments and develop tailored care plans.
  • Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, promptly report changes in condition, and coordinate care with physicians and other team members.
  • Educate patients and families about managing illnesses, medications, and preventive care.
  • Utilize Home Care Home Base (HCHB) for accurate, timely documentation.
  • Partner with therapists, social workers, and home health aides to ensure coordinated service delivery.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Comply with all regulatory and agency standards.

    • Qualifications:

  • Active RN license (in good standing) for the state of practice.
  • At least 1 year of recent field-based Home Health RN experience.
  • Proficiency with Home Care Home Base (HCHB) and OASIS required.
  • Excellent clinical judgment, assessment, and communication skills.
  • Reliable transportation, valid driver’s license, and auto insurance.
  • Ability to manage a flexible, field-based schedule independently.
  • BSN strongly preferred.
  • Experience with 25–32 point weekly caseload and knowledge of Medicare/home health compliance highly valued.

    $2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
