An exciting opportunity exists for an experienced and compassionate Home Health Registered Nurse to deliver skilled nursing care to patients in a home setting. This contract role requires a self-motivated and organized professional who is adept at managing a diverse caseload, developing individualized care plans, and fostering strong patient-family relationships.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • 13-week contract
  • Caseload: 25–32 points per week (typically 3–5 patient visits per day)
  • Responsibilities:

  • Conduct comprehensive OASIS assessments and develop personalized care plans for each patient.
  • Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
  • Continuously evaluate patient status, recognize changes in condition, and communicate promptly with interdisciplinary team members and physicians.
  • Educate patients and families on treatment plans, medications, and preventative care strategies.
  • Maintain timely, accurate documentation using Home Care Home Base (HCHB).
  • Coordinate with physical and occupational therapists, social workers, and home health aides to optimize patient outcomes.
  • Participate in case conferences, staff meetings, and ongoing professional development.
  • Ensure compliance with all regulatory requirements and agency documentation standards.
  • Desired Experience and Qualifications:

  • Active and unrestricted RN license in the state of practice.
  • Minimum of 1 year of recent home health experience (field experience preferred).
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessments required.
  • Reliable transportation, valid driver’s license, and current auto insurance.
  • Strong clinical assessment, organizational, and communication skills.
  • Ability to work independently in the field.
  • BSN strongly preferred.
  • Experience managing a comparable caseload and knowledge of Medicare/home health compliance are advantageous.
