Deliver expert patient care and support within a dynamic rehabilitation and long-term care environment. As a Registered Nurse, your expertise in RN-Rehab/LTC is required to provide comprehensive nursing services, ensuring quality patient outcomes and fostering a compassionate atmosphere.

Schedule:

  • Rotating weekends
  • Shifts vary Sunday through Saturday

    • Essential Qualifications:

  • Current, active RN license
  • Previous experience in rehabilitation and/or long-term care nursing
  • Demonstrated ability to work collaboratively within an interdisciplinary healthcare team
  • Strong assessment, clinical, and documentation skills
  • Proficiency in electronic health records and care planning
  • BLS certification (current and valid)
  • Excellent problem-solving and communication abilities

    • Key Responsibilities:

  • Deliver patient-centered care to individuals requiring rehabilitation and long-term care interventions
  • Assess, plan, implement, and evaluate individualized care plans in accordance with established standards
  • Administer medications, monitor responses, and update treatment protocols as necessary
  • Collaborate with physicians, therapists, case managers, and families to optimize care coordination
  • Educate patients and families on treatment plans, rehabilitation progress, and ongoing care needs
  • Document all nursing interventions, patient progress, and significant observations promptly and accurately
  • Uphold regulatory guidelines and facility procedures to ensure a safe, supportive environment
