As part of this essential healthcare role, you will perform comprehensive home health risk assessments for Medicare Advantage plan members residing in Crawford County, AR, including areas such as Van Buren, Alma, and Mulberry. Each assessment typically lasts about 45 minutes and may serve as an annual evaluation or a post-discharge visit, providing a valuable service to members in their own homes.

Schedule:

  • Minimum weekly or bi-weekly hourly commitment
  • Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical history, ongoing medical conditions, and medications
  • Educate members on their health conditions and medication regimens
  • Identify gaps in care and offer recommendations to address them
  • No need to develop treatment plans or prescribe medications
  • Qualifications:

  • Active Nurse Practitioner License in Arkansas (AR)
  • Current Basic Life Support (BLS/CPR) certification
  • Board Certification through AANP or ANCC required
  • Master of Science in Nursing (MSN) from an accredited institution
  • At least six months of Nurse Practitioner experience preferred
  • Nurse Practitioners in this role are compensated at $80-$90 per completed evaluation, along with a mileage stipend and paid administrative time.
