Advance your career in home health as a Nurse Practitioner conducting risk assessments on a per diem basis. This dynamic position serves Medicare Advantage plan members throughout Nez Perce County, including Lewiston, Lapwai, and Peck. Your expertise will be key in evaluating and educating individuals during their annual health assessment or post-discharge visits, helping to enhance preventive care across the community.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own working hours and calendar
  • Minimum required commitment is hourly, either weekly or bi-weekly, for added flexibility
  • Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate patients’ medical history, current conditions, and medication use
  • Educate patients on the management of medical conditions and proper medication usage
  • Identify gaps in care and offer recommendations to close them
  • No responsibility for developing treatment plans or prescribing medications
  • Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Current BLS (CPR) certification
  • Board Certification as a Nurse Practitioner through AANP or ANCC
  • Minimum of 6 months of current Nurse Practitioner experience preferred
  • MSN degree from an accredited nursing program
  • This per diem opportunity offers compensation of $80-$90 per evaluation, along with a mileage stipend and pay for administrative time.
