You will perform home health risk assessments for Medicare Advantage plan members across Elmore County, with visits potentially covering annual evaluations or post-discharge follow-ups. Each assessment averages 45 minutes, empowering you to focus on quality, not quantity.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own work schedule
  • Minimum weekly or bi-weekly hourly commitment
  • Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage members in their homes
  • Review and evaluate prior and current medical history and medications
  • Assess and identify medical conditions; educate patients about their health and medication management
  • Detect care gaps and recommend solutions
  • Note: This role requires no creation of treatment plans or prescription of medications
  • Qualifications:

  • Active Nurse Practitioner License in Idaho
  • Current Basic Life Support (BLS/CPR) certification
  • Board Certification by AANP or ANCC
  • Minimum of 6 months’ recent NP experience preferred
  • MSN degree from an accredited nursing program
  • This per diem position offers a competitive per-evaluation rate of $80 to $90, with additional compensation for mileage and administrative time.
