Take on a vital clinical role supporting patients in nursing home and assisted living environments, delivering comprehensive care that aims to minimize unnecessary hospitalizations and assist successful transitions home. This opportunity focuses on providing consistent, high-quality care to insurance members residing in skilled nursing and assisted living facilities.

Schedule:

  • Monday through Friday, 8:00 AM – 5:00 PM
  • No weekends or holidays required
  • Key Responsibilities:

  • Conduct monthly (or more frequent, as needed) health assessments and develop individualized medical treatment plans for each resident.
  • Provide on-site primary, preventive, urgent, and acute care.
  • Identify and stratify high-risk patients for prioritized, frequent clinical visits and implement contingency plans for those at elevated risk.
  • Collaborate closely with residents’ primary care physicians as well as facility staff, including Directors of Nursing.
  • Deliver direct, hands-on bedside care, manage care plans, and create medical orders.
  • Visit patients newly admitted to the facility, evaluate their needs, and facilitate safe and successful discharge planning.
  • Manage a panel of members across multiple facilities, with primary assignments scheduled at one site per day, but with flexibility to attend to acute needs at other locations when necessary.
  • Qualifications:

  • Master of Science in Nursing (MSN)
  • Active New York Nurse Practitioner (NP) license
  • Current BLS certification
  • Experience in geriatrics, skilled nursing, or assisted living care is highly valued
  • This contract position offers a competitive hourly compensation between $75 and $100 based on experience and credentials. The role provides the opportunity to make a direct, positive impact in the lives of residents by providing proactive, compassionate medical care.
