Experienced and compassionate Home Health Registered Nurses are needed to provide high-quality, patient-centered care for individuals in their homes. This contract position focuses on delivering skilled nursing services, fostering strong patient connections, and executing comprehensive care plans tailored to each individual’s unique needs. Ideal candidates are highly organized, comfortable with field-based autonomy, and thrive in collaborative, fast-paced healthcare settings.

Schedule & Structure:

  • Full-time, 13-week contract
  • Monday – Friday, 8:00 AM – 5:00 PM
  • Caseload: 25–32 points/week (typically 3–5 patient visits per day)
  • Daily travel within assigned territory (mileage reimbursement provided)
  • Field-based position (patient home visits)
  • Key Responsibilities:

  • Conduct thorough OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing interventions: medication administration, IV therapy, wound care, and chronic condition management.
  • Evaluate and monitor patient progress, promptly communicating changes to the interdisciplinary team.
  • Offer education to patients and families regarding disease processes, medication regimens, and preventive health measures.
  • Accurately document all care activities using Home Care Home Base (HCHB).
  • Collaborate with a multidisciplinary team including therapists, social workers, and aides.
  • Participate in case conferences, meetings, and ongoing training as required.
  • Ensure compliance with regulatory and agency standards for high-quality patient care.
  • Qualifications:

  • Current, active RN license in good standing in the state of practice.
  • At least 1 year of recent home health (field-based) nursing experience.
  • Proficiency in Home Care Home Base (HCHB) and OASIS.
  • Strong clinical assessment, communication, and organizational skills.
  • Reliable transportation, valid driver’s license, and auto insurance.
  • BSN and prior experience managing full caseloads are preferred.
  • Familiarity with Medicare and home health regulatory requirements.

    $2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
