An opportunity is available for an experienced and compassionate Home Health Registered Nurse (RN) to provide patient-centered care in a field-based setting. This contract position involves supporting individuals in their homes, utilizing your clinical expertise to ensure quality, continuity, and positive outcomes.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Full-time, 13-week contract

    • Key Responsibilities:

  • Conduct comprehensive OASIS assessments to develop and manage individualized care plans.
  • Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.
  • Monitor patient progress, identify changes in health status, and coordinate with physicians and interdisciplinary healthcare teams.
  • Educate patients and families about disease processes, medications, preventive measures, and treatment plans.
  • Accurately document care in Home Care Home Base (HCHB) and uphold agency and regulatory standards.
  • Collaborate with physical and occupational therapists, social workers, and home health aides to ensure integrated care.
  • Attend case conferences, staff meetings, and participate in continued education as needed.
  • Provide daily travel to patient homes within the assigned territory (mileage reimbursement provided).

    • Qualifications:

  • Active RN license in good standing for the state of practice.
  • Minimum one year of recent home health experience (field-based experience preferred).
  • Proficiency with Home Care Home Base (HCHB).
  • Experience with OASIS documentation.
  • Solid clinical assessment and critical thinking abilities.
  • Reliable transportation, valid driver’s license, and current auto insurance.
  • Ability to work independently and efficiently manage a field-based schedule.
  • Excellent organizational, communication, and time-management skills.
  • BSN and experience with 25–32 points per week preferred.
  • Familiarity with Medicare and home health compliance is a plus.

    $2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
