Advance your nursing career in a rewarding field-based position providing hands-on care to patients in their homes. As a Home Health LPN, you will play a key role in delivering quality, patient-centered care, fostering independence, and ensuring positive outcomes through skilled clinical interventions and ongoing education.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Occasional flexibility for patient needs
  • Key Responsibilities:

  • Conduct skilled nursing visits, assessing and implementing individualized care plans for a home health population
  • Administer medications, perform wound care, and manage IV therapy or other clinical interventions following physician orders
  • Monitor and report patient condition changes immediately to the RN Case Manager or physician
  • Educate patients and caregivers on medication management, disease process, and self-care techniques
  • Complete accurate and timely documentation through Home Care Home Base (HCHB)
  • Collaborate closely with the interdisciplinary healthcare team to ensure coordinated and compliant patient care
  • Adhere to infection control protocols and maintain highest standards for patient safety
  • Participate actively in case conferences, quality initiatives, and mandatory training sessions
  • Required Qualifications:

  • Current, unrestricted LPN license in Tennessee
  • At least 1 year of home health, hospice, or community-based care experience
  • Home Care Home Base (HCHB) proficiency
  • Valid driver’s license, reliable transportation, and current auto insurance
  • Current BLS certification
  • Excellent clinical judgment, organization, and communication skills
  • Ability to independently manage a caseload in a field-based environment
  • Preferred Background:

  • Experience in Medicare-certified home health agencies
  • Strong grasp of home health compliance and documentation standards
  • Outstanding interpersonal skills with a commitment to compassionate, patient-centered care
