An opportunity is available for an experienced Licensed Practical Nurse (LPN) specializing in home health care, dedicated to delivering patient-centered nursing support in a field-based setting. This contract role allows nurses to empower patients in their recovery and independence, while maintaining high standards of clinical care and compliance.

Schedule:

  • Monday to Friday, 8:00 AM – 5:00 PM (occasional flexibility to meet patient needs)
  • Field-based, serving patients in their homes within an assigned territory
  • Expected caseload: 25–32 points per week (approximately 3–5 visits per day)
  • Key Responsibilities:

  • Conduct comprehensive nursing visits to plan and implement individualized care for home health patients
  • Administer prescribed medications, wound care, and manage IV therapy or other clinical interventions in accordance with orders
  • Continuously assess and monitor patient condition, promptly reporting to the RN Case Manager or physician
  • Provide education to patients and caregivers regarding diseases, medications, and self-management
  • Maintain accurate and timely documentation of all care activities via the Home Care Home Base (HCHB) charting system
  • Collaborate effectively with the interdisciplinary healthcare team to ensure cohesive patient care and regulatory compliance
  • Uphold infection control practices and prioritize patient safety at all times
  • Attend required meetings, quality improvement initiatives, and ongoing training
  • Required Qualifications:

  • Current and unrestricted LPN license (state of practice)
  • At least 1 year of recent home health, hospice, or community-based care experience
  • Proficiency with Home Care Home Base (HCHB)
  • Active driver’s license, insured and reliable vehicle
  • BLS (Basic Life Support) certification
  • Excellent organizational, clinical, and communication abilities
  • Comfort working independently in a field-based role
  • Preferred:

  • Background in Medicare-certified home health agencies
  • Strong compliance knowledge and experience with regulatory standards
