Role Summary:
We are looking for a compassionate and detail-oriented Home Health RN to join our Crestview Hills team. This is an excellent opportunity for an RN who enjoys building meaningful patient relationships.
Responsibilities:
Perform skilled nursing visits in patient homes.
Develop and revise individualized care plans.
Provide education to patients and caregivers on health management.
Ensure timely and accurate documentation in the EMR system.
Adhere to agency policies and Medicare guidelines.
Qualifications:
Active RN license in Kentucky or compact state.
At least 1 year of home health experience, including OASIS documentation.
BLS certification.
Excellent time management and communication skills.
