School Nurse (RN/LSN/LPN) 1:1 Support | Plymouth, MN | Immediate Start through June 5, 2025

Location: Plymouth, MN (single school site)

Start Date: ASAP

End Date: June 5, 2025

Schedule: Monday–Friday, 35 hours per week

Caseload: 1:1 assignment with a medically fragile student

Student Medical Profile:

The student has multiple medical conditions, including:

Cerebral palsy spastic quadriplegia (post-selective dorsal rhizotomy)

Hydrocephalus with a nonfunctioning shunt

Oral dysphagia requiring gastrostomy tube dependency

Neuromuscular scoliosis and spastic hip dysplasia

Chronic lung disease

Global functional impairment

Responsibilities:

Monitor and adjust oxygen levels hourly

Administer G-tube feedings and medications

Provide wound care and nebulizer treatments as needed

Assist with transfers using a Hoyer lift and standing machine

Support hygiene and daily living activities

Coordinate with medical providers and the student's guardian

Accompany the student throughout the school day, ensuring safety and comfort

Prepare the student for transportation home, including oxygen checks

Qualifications:

Active RN, LSN, or LPN license in Minnesota

Previous experience in a school setting is required

Experience with 1:1 care for medically fragile students is preferred

LPN candidates must have multiple years of relevant school experience

Proficiency in managing complex medical needs in a school environment
