Your next nursing opportunity is here: A dedicated Registered Nurse with demonstrated OASIS experience is needed for a full-time, 13-week home health contract. Schedule:

  • 13-week contract with potential extension
  • Full-time hours, Monday through Friday
  • On-call responsibilities may be required

    • Key Responsibilities:

  • Deliver skilled nursing care in patient homes while ensuring high standards of safety and quality
  • Perform patient assessments and complete OASIS documentation accurately and in a timely manner
  • Collaborate closely with physicians and interdisciplinary teams to create and implement individualized care plans
  • Carry out wound care, medication administration, patient education, and disease management
  • Communicate effectively regarding patient status and any changes to the wider clinical team

    • Required Qualifications & Experience:

  • Current, active RN license
  • At least one year of recent RN experience specifically in home health
  • Demonstrated experience with OASIS documentation
  • Strong preference for proficiency with HomeCare HomeBase software
  • Capable of managing a structured daily schedule with multiple patient visits
  • Reliable transportation and a valid driver’s license are essential
