This contract position focuses on delivering compassionate and comprehensive nursing care to patients in their homes, helping them achieve the best possible outcomes while maintaining comfort and independence.

Key Responsibilities:

  • Provide hands-on nursing care to home health patients, including initial assessments, vital signs, medication management, and wound care
  • Utilize OASIS for accurate assessments and care planning
  • Develop and implement individualized care plans that address each patient's unique health needs
  • Educate patients and their families on managing symptoms, treatment regimens, and preventive health practices
  • Collaborate closely with physicians, physical therapists, and other interdisciplinary care team members to ensure well-coordinated care
  • Accurately document all patient interactions and clinical interventions in electronic medical records (EMR) systems
  • Communicate any significant patient status changes to supervising physicians promptly
  • Adhere to all regulatory guidelines and best practices for home health care
  • Qualifications:

  • Current, active RN license
  • At least 3 months of home health experience utilizing OASIS
  • Familiarity with HCHB (Homecare Homebase) is highly desirable
  • Strong clinical assessment skills, including IV therapy and wound care
  • Proficiency in EMR systems
  • Outstanding communication, organization, and time-management abilities
  • Self-motivated with the capability to work independently
  • Valid driver’s license and reliable transportation
