Deliver patient-centered care in skilled nursing and assisted living environments, focusing on improving the quality of life for residents and reducing avoidable hospital admissions. This role supports comprehensive care programs aimed at treating patients in place whenever possible, ensuring smooth transitions from facility to home and promoting optimal health outcomes for insurance members.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • No weekends or holidays
  • Key Responsibilities:

  • Conduct regular patient rounds in multiple facilities, ensuring health needs are met and documented thoroughly.
  • Perform routine monthly assessments for residents, as delegated by the Nurse Practitioner—frequency may increase as clinical needs dictate.
  • Review each resident’s health status and collaborate on care plans to achieve established patient goals.
  • Represent the Nurse Practitioner at facility meetings, provide updates, and effectively communicate information to the Interdisciplinary Care Team.
  • Carry out care coordination activities, actively supporting successful transitions of care and reducing unnecessary emergency room visits or hospitalizations.
  • Qualifications:

  • Active RN license in the state of New York (BSN or ASN required)
  • Current Basic Life Support (BLS) certification
  • Proven expertise in long-term care settings, skilled nursing, or assisted living
  • Ability to coordinate complex care needs and communicate effectively within multi-disciplinary teams
  • Strong commitment to patient advocacy and continuity of care
  • This contract opportunity offers competitive hourly compensation, between $65 and $80 per hour, reflecting both your experience and impact on resident health outcomes.
