We’re seeking a dedicated and compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to join a progressive, community-based healthcare team delivering behavioral health services in a hybrid setting throughout Buffalo, NY. This opportunity is ideal for a PMHNP who’s passionate about expanding access to care for individuals who face barriers to traditional in-office mental health support. This is a 13-week contract with 4-week extension options and a possibility for permanent hire.

Position Summary:
In this role, the PMHNP will work within a multidisciplinary team to deliver high-quality psychiatric evaluations, medication management, and mental health support to patients in home-based and virtual settings. You’ll have the chance to build meaningful therapeutic relationships with patients and make a lasting impact on mental health outcomes in Buffalo.

Key Responsibilities:

  • Conduct psychiatric evaluations, mental health assessments, and medication reviews

  • Provide diagnosis, treatment planning, and medication management for individuals with a variety of psychiatric conditions

  • Deliver care in patients’ homes and through telepsychiatry platforms

  • Collaborate with primary care providers, social workers, therapists, and care coordinators

  • Maintain thorough documentation in the EMR and support compliance with regulatory standards

  • Participate in case reviews, quality initiatives, and care planning sessions with the clinical team

  • Educate patients and caregivers about treatment plans, medication side effects, and self-management techniques

    Schedule & Logistics:

  • Location: Buffalo, NY, and surrounding areas

  • Schedule: Monday through Friday, 8:00 am – 5:00 pm

  • Travel: Regular in-home visits required (reliable transportation needed)

  • Setting: Hybrid – mix of in-home visits and virtual sessions

  • Contract: 13 weeks with option to extend every 4 weeks; possible permanent hire

    Qualifications:

  • Active, unrestricted PMHNP license in New York 

  • Board Certification in Psychiatric-Mental Health (ANCC)

  • Valid DEA, NPI, and current BLS certification

  • Experience in outpatient, community, or home-based mental health settings

  • Reliable transportation and valid auto insurance

  • Proficiency in EMR systems; Medent experience a plus

    Preferred:

  • Experience working with dual-diagnosis populations (mental health + substance use)

  • Knowledge of trauma-informed care, SPMI treatment models, and behavioral care coordination

  • Bilingual capabilities are a plus

    Compensation & Benefits:

  • Hourly Rate: Negotiable with client; typical range $85–$95/hr
