Healthcare professionals with experience in home health are invited to join a dynamic per diem opportunity providing vital risk assessments for Medicare Advantage plan members in Woodruff County, Arkansas. This role is ideal for nurse practitioners interested in independent, patient-focused work with flexibility and autonomy.

Schedule:

  • Flexible shifts available Monday through Sunday

  • Minimum weekly or bi-weekly hourly commitment required
  • Responsibilities:

  • Conduct home health risk assessments, including annual health evaluations and post-discharge visits, in various locations within Woodruff County such as Augusta, McCrory, and Cotton Plant
  • Complete thorough patient reviews, evaluating past and current medical history, existing medical conditions, and detailed medication histories
  • Deliver patient education regarding medical conditions and medications
  • Identify gaps in care and recommend appropriate next steps or referrals
  • No responsibilities for developing treatment plans or prescribing medication
  • Desired Experience & Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • MSN degree from an accredited school of nursing
  • Board certification through AANP or ANCC
  • Current CPR (BLS) certification
  • At least 6 months of experience as a Nurse Practitioner is preferred
  • In this position, you will earn $80-$90 per evaluation, alongside a mileage stipend and compensation for administrative time. The average assessment takes approximately 45 minutes and scheduling is based on the needs of members throughout the region.
