Step into a pivotal healthcare role providing comprehensive in-home assessments for Medicare Advantage members in Minidoka County. As an experienced Nurse Practitioner specializing in home health, you will have the opportunity to deliver patient-centered care and meaningful education directly in the comfort of clients’ residences. This per diem position offers exceptional autonomy with a fully flexible schedule, allowing you to tailor your shifts anywhere between Monday and Sunday, and meet minimum weekly or bi-weekly hourly commitments based on your availability.

Key Responsibilities:

  • Conduct annual health risk assessments and post-discharge evaluations in-home for Medicare Advantage members.
  • Review and evaluate prior and current medical histories, medical conditions, and medication usage.
  • Educate members on health conditions and medications, empowering informed decisions.
  • Identify care gaps, collaborating with care teams to provide recommendations for follow-up.
  • Enjoy a clinical role focused on assessment and education, with no need to create treatment plans or prescribe medications.

  • Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited institution
