Schedule:

  • In this position, you'll conduct comprehensive home health risk assessments for individuals in their residences. Each visit typically lasts around 45 minutes and may include annual assessments or post-discharge check-ins. Your clinical insights will help evaluate medical histories, review current medications, identify care gaps, and provide education—without the need to develop treatment plans or prescribe medications.

    Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Board Certification via AANP or ANCC
  • Current CPR certification (BLS required)
  • MSN degree from an accredited nursing program
  • At least 6 months of Nurse Practitioner experience preferred
  • Responsibilities:

  • Perform annual home health risk assessments exclusively for Medicare Advantage plan members
  • Assess prior and current medical conditions, medication usage, and educate patients accordingly
  • Detect care gaps and recommend appropriate next steps
  • Deliver thorough, patient-centered documentation and communicate findings
  • Collaborate with the healthcare team as needed, while remaining focused on assessment details
  • This per diem role offers competitive pay of $80-$90 per evaluation, plus a mileage stipend and additional compensation for administrative work.
