This role is centered on conducting comprehensive home health risk assessments in Idaho County, serving communities in and around Grangeville, Cottonwood, and Kamiah. Assessments generally last about 45 minutes and may include annual evaluations or post-discharge visits, helping ensure continuity of care for members in their own homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling allows for creating your own workdays
  • Commitment to a minimum number of hours weekly or bi-weekly
  • Key Responsibilities:

  • Complete annual risk assessments for Medicare Advantage members at their residences
  • Review and document prior and current medical history, underlying conditions, and medication regimens
  • Educate members on their health conditions and medication management
  • Identify and report care gaps, offering actionable recommendations
  • No requirement for developing treatment plans or writing prescriptions
  • Qualifications:

  • Current and unrestricted Nurse Practitioner license in Idaho
  • Board Certified by the AANP or ANCC
  • Active CPR certification (BLS)
  • Minimum of 6 months of recent NP experience preferred
  • Master of Science in Nursing from an accredited program
  • Competitive compensation is offered, with $80–$90 for each evaluation completed, plus a mileage stipend and pay for administrative time.
