Shift: Monday-Friday, with possible on-call

Job Description:
Sunbelt Staffing is seeking an experienced Home Health Registered Nurse (RN) for a 13-week contract in Holyoke, MA. This position is ideal for RNs proficient in OASIS documentation and experienced with Homecare Homebase, dedicated to delivering high-quality patient care in a home health setting.

Key Responsibilities:

  • Conduct thorough patient assessments and ensure accurate OASIS documentation
  • Develop and oversee personalized care plans based on individual patient needs
  • Provide skilled nursing services, including wound care, medication management, and health education
  • Work collaboratively with an interdisciplinary team to deliver seamless patient care
  • Educate patients and families on care plans, health conditions, and recovery goals

    Qualifications:

  • Active RN license in Massachusetts
  • Strong proficiency in OASIS documentation and Homecare Homebase required
  • Minimum of 2 years of Home Health nursing experience preferred
  • Ability to pass background check, drug screening, and onboarding in compliance with Joint Commission standards
  • Open to local and travel candidates
