Responsibilities:

  • Provide comprehensive nursing care to patients in a hospice setting
  • Assess and monitor patients' physical and emotional well-being
  • Develop and implement individualized care plans based on patient needs
  • Administer medications and treatments as prescribed
  • Collaborate with interdisciplinary teams to ensure holistic patient care
  • Provide education and support to patients and their families
  • Maintain accurate and up-to-date patient documentation

    Qualifications:

  • Valid and current Registered Nurse (RN) license
  • Minimum of 6 months of experience in hospice
  • Proficiency in using electronic medical record systems, with experience in HCHB preferred
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal skills
  • Compassionate and empathetic attitude towards patients and their families
  • Valid driver's license and reliable transportation
