Delivering compassionate hospice care requires a blend of clinical expertise, empathy, and collaboration. This vital field-based position calls for an experienced Registered Nurse ready to provide exceptional end-of-life care and holistic support to patients and their families. The contract spans 13 weeks with a full-time schedule—Monday through Friday, 8:00 AM to 5:00 PM—with occasional on-call or weekend responsibilities.

Responsibilities include:

  • Conducting nursing assessments and developing individualized hospice care plans
  • Delivering skilled nursing care focused on comfort, pain management, and symptom control
  • Administering medications and interventions according to hospice protocols and physician orders
  • Monitoring changes in patient conditions and coordinating timely communication with the medical director and care team
  • Providing education and emotional support to patients, family members, and caregivers
  • Collaborating with interdisciplinary teams (including social workers, chaplains, and aides) to address comprehensive care needs
  • Accurately completing documentation in Home Care Home Base (HCHB) and ensuring compliance with all hospice policies and regulatory guidelines
  • Participating in case conferences, team meetings, and continuous quality improvement
  • Managing daily travel to patient locations within the assigned service area
  • To thrive in this role, you’ll bring:

  • An active RN license (in good standing)
  • At least one year of hospice nursing experience
  • Proficiency in Home Care Home Base (HCHB) software
  • Strong clinical assessment, communication, and teaching skills
  • Reliable transportation, a valid driver’s license, and current auto insurance
  • Resilience, compassion, and a commitment to high-quality end-of-life care
  • BSN degree and prior palliative care experience preferred

    $2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
