Home Health Registered Nurse (RN) – Contract
Location: Lavonia, GA
Schedule: Monday to Friday, 8:00 AM – 5:00 PM
Duration: 13 Weeks
Hours: 40 hours/week (No weekends, No call)
Overview:
A respected home health agency in the Lavonia area is looking for a dedicated and experienced Registered Nurse to join their care team for a 13-week contract. This position involves providing skilled nursing visits in patients' homes, coordinating care, and ensuring quality outcomes in a community-based setting.
Requirements:
Active RN license in Georgia
At least 1 year of recent home health nursing experience
Proficient in OASIS documentation (expectation: 24–48 hour turnaround)
Experience with HomeCare HomeBase (HCHB) strongly preferred
Reliable transportation and valid driver’s license
Strong clinical assessment and case management skills
Key Responsibilities:
Deliver direct patient care in the home setting
Perform comprehensive assessments and develop individualized care plans
Educate patients and caregivers on disease management and medication administration
Complete OASIS-E documentation accurately and on time
Coordinate with physicians, therapists, and other healthcare providers
