Job Description:
Sunbelt Staffing is hiring a skilled Home Health Registered Nurse (RN) for a 13-week contract in Fort Wayne, IN. This role is designed for nurses with expertise in OASIS documentation and Homecare Homebase, ready to deliver top-quality care to patients in their homes.

Key Responsibilities:

  • Perform detailed patient assessments and complete OASIS documentation accurately
  • Create and implement tailored care plans to meet individual patient needs
  • Provide essential nursing care, including wound care, medication administration, and health education
  • Collaborate with healthcare teams to ensure seamless and effective patient care
  • Support patients and their families in understanding care plans and recovery steps

    Qualifications:

  • Active RN license in Indiana
  • Proficiency in OASIS documentation and Homecare Homebase required
  • Minimum of 2 years of Home Health nursing experience preferred
  • Ability to pass background check, drug screening, and onboarding in compliance with Joint Commission standards
  • Open to both local and travel candidates
