As a Home Health Registered Nurse, you will be instrumental in providing skilled nursing services to patients in their homes, supporting continuity and quality throughout the care journey.

Schedule:

  • Full-time, contract position (13-week duration)
  • Monday – Friday, 8:00 AM – 5:00 PM
  • Caseload of 25–32 points/week (typically 3–5 patient visits per day)
  • Daily travel within assigned territory (mileage reimbursement provided)

    • Key Responsibilities:

  • Complete thorough OASIS assessments and develop individualized plans of care
  • Administer skilled nursing interventions, including wound care, IV therapy, medication management, and chronic disease monitoring
  • Track patient progress, identify status changes, and coordinate with clinical teams and physicians
  • Educate patients and families about disease processes and care management
  • Accurately document clinical visits in Home Care Home Base (HCHB)
  • Collaborate with interdisciplinary team members to ensure quality, coordinated care
  • Participate in case conferences, team meetings, and ongoing training
  • Uphold regulatory and organizational standards for all patient care and documentation

    • Qualifications:

  • Active RN license (state of practice)
  • At least 1 year of recent home health nursing (field experience preferred)
  • Proficiency with Home Care Home Base (HCHB) and OASIS required
  • Strong clinical assessment, organization, and communication skills
  • Valid driver's license, reliable transportation, and auto insurance
  • Ability to manage a flexible field-based schedule
  • BSN and experience managing 25–32 point caseloads preferred
  • Knowledge of Medicare and home health compliance a plus

    $1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
