Job Title: Home Health Licensed Practical Nurse (LPN)
Location: Jeffersonville, IN

Duration: 13 weeks

Shift: Monday-Friday, with potential on-call

Job Description:
Sunbelt Staffing is hiring a dedicated Home Health Licensed Practical Nurse (LPN) for an assignment in Jeffersonville, IN. In this role, you'll provide high-quality, personalized care to patients in their homes, manage a diverse caseload, and ensure accurate documentation using Homecare Homebase. Key Responsibilities:

  • Provide skilled nursing care based on individualized home health care plans.
  • Administer medications, perform wound care, and educate patients and families.
  • Monitor patient conditions and update care plans as needed.
  • Document all patient care accurately and efficiently using Homecare Homebase.
  • Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care.

    Qualifications:

  • Active LPN license in Indiana or compact state licensure.
  • At least 3 years of LPN experience, with a minimum of 1 year of recent home health care experience.
  • Proficiency with Homecare Homebase is required.
  • Current BLS certification.
  • Reliable transportation and a valid driver’s license.
  • Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission.
