Shift: Monday-Friday, with potential on-call

Job Description:
Sunbelt Staffing is looking for an experienced Home Health Registered Nurse (RN) to join our team for a 13-week contract in Fort Wayne, IN. This role requires proficiency in OASIS documentation and experience with Homecare Homebase to ensure the delivery of exceptional patient care in a home health setting.

Key Responsibilities:

  • Conduct patient assessments and complete precise OASIS documentation
  • Develop and manage personalized care plans for patients
  • Deliver skilled nursing care, including wound care, medication administration, and health monitoring
  • Collaborate with interdisciplinary teams to provide comprehensive patient care
  • Educate patients and families on health conditions, recovery processes, and self-care

    Qualifications:

  • Active RN license in Indiana
  • Proficiency in OASIS documentation and experience with Homecare Homebase required
  • Minimum of 2 years of Home Health nursing experience preferred
  • Ability to pass background check, drug screening, and onboarding in compliance with Joint Commission standards
  • Open to both local candidates and travelers
